Health Ministries Clinic, Newton, is bringing a mobile unit to the Hesston High School campus to provide FREE sports physicals for students, age 13 and older. Please read the important details below.
Date/Time of Clinic: Wednesday, May 8, 8:30 a.m.-4:35 p.m. All appointments will be scheduled through the middle school (HMS) and high school (HHS) offices.
Documents Required: Parents/guardians wishing for their student to receive a free sports physical at this clinic on May 8 must provide a signed consent form and a KSHSAA Pre-Participation Physical Evaluation form (available below) to the student's school office BY FRIDAY, MAY 3. These forms MUST BE DATED May 1, May 2, or May 3, 2024.
Note: No physical will be conducted unless BOTH required forms are submitted on/by MAY 3.
